Surgical Abdomen in Patients With Blood Dyscrasia



Abstract
During the 6 years 6 months period from january 1985 to july 1991, 12 patients with blood dyscrasia(hemophilia, leukemia, aplastic anemia, glnzmann's thrombasthenia and hereditary spherocytosis), admitted and operated at Department of surgery,
college
of medicine, Chonnam national university. And we analysed and represented about preoperative diagnosis, operative finding, postoperative complication especially infection and hemonhage.
1) The view that surgery is absolutely contraindicated in leukemia and aplastic anemia with or without thrombocytopenia and leukopenia is no longer justified, Prophylactic platelet therapy and proper antibiotic administration does permit surgery
with
an acceptable morbidity and mortality in these patients.
2) Surgical procedure on patients with congenital disorders of blood coagulation, after preoperative screening test such as partial thromboplastin time(PTT), factor assay, careful hemorrhagic management including specific factor replacement
poses
few
untoward problems.
3) Surgical principles apply in patients with defect in blood cogulation, careful intraoperative hemostasis is of primary importance. Meticulous clamping and ligation of all beeding points with minimal use of cautery, gentle handling of tissues,
minimal procedure and elimination of dead space are important to minimized postoperative hemorrhagic complication.
In conclution, surgery, in patients with blood dyscrasia, is not absolutely contraindicated, With accurate preoperative diagnosis, replacement therapy permit acceptable morbidity and mortality in thease patients.
